Rules for meld sets?

I'm a little confused by a specific rule on the meld sets. I've noticed that the first and last cards in a sequence can sometimes be melded with a set. Why can I sometimes use the card of the same suit and rank in two different melds, and sometimes I can't? I'm sorry for the dumb question, but I'm just trying to understand as the rules don't really explain that nuance.

If you are holding 3456 diamonds and 6heart and 6spade. = 345 diamonds and 666 ! Lower your count. 2 spreads? Is this the question
